Ultrasonic assessment of cesarean section scar to vesicovaginal fold distance: an instrument to estimate pre-labor uterine rupture risk
Background: The number of Cesarean sections (CS) is growing worldwide, intensifying the risk of complications in subsequent pregnancies and leading to increased maternal and fetal morbidity and mortality. In particular, the literature shows a higher risk of uterine rupture (UR) in subsequent pregnancy with trial of labor after cesarean section (TOLAC) Furthermore, there are few data about pre-labor UR in scarred uteri. Objective: Since the key factor for management is timing, the aim of this study was to evaluate the accuracy of prenatal ultrasound (US) of scars in the early determining of pre-labor UR risk in women with a previous CS during their subsequent pregnancy Methods: From April 2014 to November 2018 a retrospective analysis was performed in order to evaluate the scar to vesicovaginal fold (VVF) distance in three patients with pre-labor UR and in 60 cases of the control group. Results: The periconceptional CS scar-VVF distance in the three UR cases resulted significantly increased compared to the controls (23.7 ± 3.5 mm vs 2.3 ± 2.7 mm, p < 005); moreover, a time interval of less than 18 months and a previous pre-labor preterm CS were found as known risk factors. Conclusion: In this study, a higher uterine incision due to placenta previa or isthmic myoma seems to be correlated with a major risk of UR. Therefore, periconceptional US examination of CS-VVF distance, (which represents the level of the previous CS), seems to be a useful predictive factor of pre-labor UR in subsequent pregnancies
Laminar Necrosis and Hypoxic Damage of the Placenta: A Case-Control Study
The aim of this study is to verify the role of laminar necrosis (LN) in the diagnosis of hypoxic damage of the placenta. This is a retrospective case-control study in which 50 cases with laminar necrosis were compared with 100 gestational age-matched controls without laminar necrosis in a 1:2 ratio. The parameters analyzed were: the presence of other placental lesions, obstetric characteristics and neonatal outcome. For each of the 50 cases, the area affected by the lesion was detected, and the lesions were classified into three groups based on the morphology and time of onset of the lesion in order to understand whether these characteristics of the lesion had a clinical-pathology. The results showed that including the search for LN among placental lesions generally examined is useful to guide the pathologist in the diagnosis of placental dysfunction of hypoxic origin

Dynamic Allocation for Resource Protection in Decentralized Cloud Storage
Decentralized Cloud Storage (DCS) networks represent an interesting solution for data storage and management. DCS networks rely on the voluntary effort of a considerable number of (possibly untrusted) nodes, which may dynamically join and leave the network at any time. To profitably rely on DCS for data storage, data owners therefore need solutions that guarantee confidentiality and availability of their data. In this paper, we present an approach enabling data owners to keep data confidentiality and availability under control, limiting the owners intervention with corrective actions when availability or confidentiality is at risk. Our approach is based on the combined adoption of AONT (All-Or-Nothing-Transform) and fountain codes. It provides confidentiality of outsourced data also against malicious coalitions of nodes, and guarantees data availability even in case of node failures. Our experimental evaluation clearly shows the benefits of using fountain codes with respect to other approaches adopted by current DCS networks
Development of lens opacities with peculiar characteristics in patients affected by thalassemia major on chelating treatment with deferasirox (ICL670) at the Pediatric Clinic in Monza, Italy.
About the 11–14% of patients with thalassemia major (TM) treated with deferasirox (DFO) develops retinopathy and/or lens opacities with an unclear pathogenesis but with a clear age related pattern.[1][1],[2][2] Possible causes can be either iron overload itself or DFO toxicity, with variou
Quantitative Effects of Repeated Muscle Vibrations on Gait Pattern in a 5-Year-Old Child with Cerebral Palsy
Objective. To investigate quantitatively and objectively the effects of repeated muscle vibration (rMV) of triceps surae on the gait pattern in a 5-year-old patient with Cerebral Palsy with equinus foot deformity due to calf spasticity. Methods. The patient was assessed before and one month after the rMV treatment using Gait Analysis. Results. rMV had positive effects on the patient's gait pattern, as for spatio-temporal parameters (the stance duration and the step length increased their values after the treatment) and kinematics. The pelvic tilt reduced its anteversion and the hip reduced the high flexion evidenced at baseline; the knee and the ankle gained a more physiological pattern bilaterally. The Gillette Gait Index showed a significant reduction of its value bilaterally, representing a global improvement of the child's gait pattern. Conclusions. The rMV technique seems to be an effective option for the gait pattern improvement in CP, which can be used also in very young patient. Significant improvements were displayed in terms of kinematics at all lower limb joints, not only at the joint directly involved by the treatment (i.e., ankle and knee joints) but also at proximal joints (i.e., pelvis and hip joint)
A Declarative Framework for Specifying and Enforcing Purpose-aware Policies
Purpose is crucial for privacy protection as it makes users confident that
their personal data are processed as intended. Available proposals for the
specification and enforcement of purpose-aware policies are unsatisfactory for
their ambiguous semantics of purposes and/or lack of support to the run-time
enforcement of policies.
In this paper, we propose a declarative framework based on a first-order
temporal logic that allows us to give a precise semantics to purpose-aware
policies and to reuse algorithms for the design of a run-time monitor enforcing
purpose-aware policies. We also show the complexity of the generation and use
of the monitor which, to the best of our knowledge, is the first such a result
in literature on purpose-aware policies.Comment: Extended version of the paper accepted at the 11th International

Peri-Conceptional Intake of Folic Acid Supplement to Date: A Medical-Legal Issue
Folic Acid (FA) supplementation during pregnancy represents a so widespread and established recommendation all over the world, to be taken for granted sometimes. As a matter of fact, this vitamin supplement is worldwide recommended mostly during peri-conceptional period for its proved preventive effect on Neural Tubal Defects (NTDs), like spina bifida. However, The biological and clinical potential of FA is reassessing and this represents a hot topic in scientific community, mostly in consideration of the possible medical-legal implications. An overview is mandatory in order to keep in mind FA-related possibl
Multi-Provider Secure Processing of Sensors Data
We describe the implementation of an approach for supporting secure query processing over sensors data in a multi-provider scenario. Our solution relies on the definition of authorizations regulating access to data according to three different visibility levels (no visibility, encrypted visibility, and plaintext visibility). Data processing is performed by multiple providers based on the restrictions imposed by authorizations, which may require to adjust data visibility on the fly. We describe the structure of the query optimizer and show how the operations of a computation can be assigned to different cloud providers to build an efficient, secure, and economical plan for collaborative data processing
Detection of Antibodies to Human Herpesvirus 8 (HHV-8) among Women of Child-Bearing Age in the Apulia Region (South-Eastern Italy):
In this study the authors investigated the presence of serum antibodies to Human Herpesvirus 8 (HHV-8) in a group of women of child-bearing age in the Apulia region (South Eastern Italy). A seroprevalence of 16.8% was observed, increasing, although non significantly, with the age of the women (10.6% in women between 19 and 25 years, 25.3% in women aging more than 35 years). The presence of antibodies to Hepatitis C Virus (HCV) was significantly associated to the detection of antibodies to HHV-8. Possible mother-to-child transmission of HHV-8 as well as the outcome of fetuses or children born to HHV-8 positive mothers are still a matter of debate. This study, showing the wide diffusion of HHV-8 infection in healthy women of child-bearing age in our geographical area, highlights the urgency of studies aimed to better clarify these relevant topics
